Enrollment Instructions

We are excited to welcome your child to Minot Children’s Academy! To begin the enrollment process, we ask for some important information about you and your child. This information is required to meet state regulations and allows us to better care for and nurture your child.

To enroll at Minot Children’s Academy:

Please complete all enrollment requirements, which include:

• Parent Sheet and Child Sheet
  These two forms must be filled out in full. You may either:
  • Print and deliver them to our location at 3 Main St S #3, Minot, ND 58701
  • Or complete them digitally, save as PDFs, and email them to minotchildrensacademy@gmail.com with the subject line: “Enrollment Files – Your Name”
